The Social Work Department is here to assist patients, their families and the community by providing:

• information and referral for home care services, such as home health agencies, hospice, delivered meal services, Lifeline, medical equipment, oxygen and so forth
   
• information and arranging assisted living, skilled care and nursing home care, as needed
   
• information about possible financial resources if finances are a concern
   
• information about and assistance in completing Advance Directives, which includes Living Wills and Durable Power of Attorney for Health Care Decisions, or information concerning Organ Donation
   
• emotional support during a difficult medical crisis
